What are Festoons?

Festoons, also called malar bags, are swollen pouches of skin and swollen tissue that sit below the lower eyelid, on the upper cheek. They often look like a sagging hammock or a fold that hangs over the cheekbone. Unlike typical under-eye bags, which form when fat pushes forward from behind the lower lid, festoons develop lower on the face and are made up of loose skin, weakened muscle, and trapped fluid. Festoons form when the skin and the orbicularis muscle around the eye lose their strength and elasticity. Over time, a pocket develops that allows fluid to collect, so festoons often look worse in the morning, after a salty meal, or during allergy season.

Common contributing factors include aging, sun damage, genetics, smoking, history of dermal filler injections and thyroid conditions. In some younger patients, fillers injected to correct  under-eye dark circles or hollowness can cause or exacerbate festoons. Because festoons involve several layers of tissue, they are among the most difficult concerns to treat around the eyes. Creams, cool compresses, and standard lower eyelid surgery alone often do not correct them. As an oculoplastic surgeon who focuses exclusively on the eyelids and face, Dr. Ghafouri has the specialized training to identify festoons, separate them from other under-eye concerns, and design a treatment plan that addresses the true cause.

Would You Benefit from Festoon Treatment?

Treatment for festoons can help patients bothered by persistent puffiness or swelling on the upper cheeks that makes them look tired, older, or unwell. Ideal candidates are in good overall health, are nonsmokers, and have realistic expectations about what treatment can achieve. Any underlying causes, such as thyroid disease or chronic allergies, should be well managed before treatment.

Festoon Treatment Can Be Used to Improve:

  • Puffy mounds or pouches on the upper cheeks
  • Swelling below the lower eyelids that worsens in the morning
  • Sagging or crepey skin between the lower lid and cheek

Extended Lower Blepharoplasty with Festoon Repair

For moderate to advanced festoons, Dr. Ghafouri may recommend an extended lower blepharoplasty. An incision is concealed just below the lash line, and the festoon is carefully addressed beneath the skin. The weakened orbicularis muscle is lifted and secured, loose skin is trimmed, and the pocket that traps fluid is reduced. When the lower lid is lax, the outer corner of the eyelid may also be tightened with canthoplasty to support the lid and protect its natural shape.

Cheek Lift

When festoons are combined with descent of the midface, a cheek lift can raise the cheek tissues back to a more youthful position. This smooths the transition between the lower eyelid and cheek and helps soften the hollows and folds that make festoons more visible.

CO2 Laser Resurfacing / Radiofrequency Micro-needling

For milder festoons, or as a complement to surgery, CO2 laser resurfacing tightens thin, crepey skin by stimulating new collagen production. Treating the skin surface helps improve texture and firmness over the festoon and can enhance surgical results. A series of radiofrequency microneedling with Morpheus8 may also be considered for select patients with early skin laxity.

Festoon Treatment Recovery

Recovery depends on the techniques used in your treatment plan. After surgical festoon repair, plan to take about 5 to 7 days off work, and limit your activity as much as possible for the first two days. Swelling and bruising usually peak during the first few days and improve gradually over the following weeks. Because festoons involve fluid in the tissues, some mild swelling on the cheeks can take longer to fully settle than it does after a standard eyelid lift.

Patients should avoid strenuous exercise and direct heat, including sun exposure and saunas, for about two weeks. Sleeping with your head elevated and applying cool compresses as directed can help reduce swelling. Make sure to follow all post-operative instructions from Dr. Ghafouri and attend your scheduled follow-up appointments so that he can monitor your healing. If laser resurfacing is part of your plan, Dr. Ghafouri will give you specific skin care instructions to support your recovery.

Frequently Asked Questions About Festoons Treatment

What is the difference between festoons and under-eye bags?

Under-eye bags sit directly beneath the lower lashes and are usually caused by fat that bulges forward from behind the eyelid. Festoons sit lower, on the upper cheek, and are made up of loose skin, weakened muscle, and fluid. Many patients have both. Because the causes are different, the treatments are different too, which is why an accurate diagnosis from an oculoplastic surgeon is imperative.

What causes festoons?

Festoons develop as the skin and muscle below the eye lose strength and elasticity, creating a pocket where fluid can collect. Aging, sun exposure, genetics, smoking, thyroid conditions, allergies, and a high-salt diet can all contribute. In some cases, under-eye filler can trigger or worsen festoons.

Can festoons go away on their own?

Festoons do not usually resolve without treatment. Reducing salt, sleeping with your head elevated, and managing allergies may temporarily lessen the swelling, but the loose skin and muscle that form the festoon remain. Professional treatment is typically needed for a lasting improvement.

Can dermal fillers injections under eye and cheek cause festoons or Malar edema?

Yes, dermal fillers injected under the eyes or into the cheeks can cause or worsen malar edema, malar bags, and festoons. Hyaluronic acid (HA) fillers are commonly associated with this complication because they attract and retain water. Fillers can also interfere with the delicate lymphatic drainage system around the lower eyelids and upper cheeks, causing fluid to accumulate. Swelling may develop immediately after injection or appear months or even years later. Excessive filler volume, improper placement, preexisting lymphatic dysfunction, and age-related skin laxity increase the risk.nnTreatment depends on the underlying cause and type of filler used. For HA fillers, hyaluronidase injections are often the first treatment considered because they dissolve retained filler and may improve lymphatic drainage and swelling. Patient may require a series of hyalurodinase injections. Additional measures may include lymphatic drainage, reducing fluid retention, and addressing inflammation when present. nnNon-HA fillers, such as Sculptra and Radiesse, cannot be dissolved with hyaluronidase and require different approaches. Early identification of filler-related malar edema is important, since prolonged swelling can be difficult to treat.

What is the difference between festoons and malar edema

Malar edema and festoons are related but distinct conditions that affect the area beneath the eyes and along the upper cheeks. Malar edema primarily involves fluid accumulation in the upper cheek area, resulting in puffiness or swelling. It may be associated with impaired lymphatic drainage, inflammation, or other factors that contribute to fluid retention. Festoons, on the other hand, are more complex and typically involve a combination of skin laxity, changes in the underlying muscle, and sometimes chronic fluid accumulation. They often appear as folds or sagging pouches along the upper cheeks/ eyelid junction.rnAlthough these conditions can look similar and may coexist, their underlying causes are different, and treatment should be tailored accordingly. Malar edema may respond to treatments that address fluid retention and lymphatic drainage, whereas festoons may require approaches that target skin laxity, structural changes, and persistent swelling. Accurately identifying the factors contributing to each condition is essential for selecting the most appropriate treatment and achieving the best possible outcome.

Will a regular lower blepharoplasty fix festoons?

A standard lower blepharoplasty is designed to address under-eye bags and is often not enough to correct festoons on its own. Festoons usually require a modified surgical approach, such as muscle suspension and festoon repair, sometimes combined with a cheek lift or laser resurfacing. Dr. Ghafouri tailors the technique to each patient.

Can fillers help with festoons?

Fillers are generally not recommended for festoons. Because festoons involve trapped fluid, adding filler to the area can draw in more fluid and make the puffiness worse. If filler has contributed to your festoons, Dr. Ghafouri can evaluate whether it should be dissolved as part of your treatment plan.

Is festoon treatment painful?

Most patients experience only mild discomfort after treatment. Surgical procedures are performed with local anesthesia or IV sedation, so you will be comfortable during the procedure.

How long do the results last?

Results from surgical festoon repair are long-lasting. You will continue to age naturally, but the treated area should look smoother and more rested for many years. Protecting your skin from the sun, avoiding smoking, not sleeping face down and maintaining a healthy lifestyle help preserve your results. As mentioned before in certain cases complete resolution of festoons could be very challenging and may need additional treatments to maintain results.

How much does festoon treatment cost?

The cost of festoon treatment depends on the techniques used and whether additional procedures, such as canthoplasty, a cheek lift, blepharoplasty, fat transfer, or laser resurfacing, are included. Every plan is personalized, so we will provide a detailed estimate after your consultation. We also offer financing options to help make treatment more accessible.

What are the possible risks of festoon treatment?

As with any procedure, general risks include infection, bleeding, poor healing, and scarring. Risks specific to festoon repair include prolonged swelling, recurrence of festoons, changes in the shape or position of the lower eyelid, dry eye, and asymmetry. In the hands of an experienced oculoplastic surgeon, these risks are minimized through careful planning and precise technique.
